Dog Heaven, by Cynthia Rylant (she also had Cat Heaven)

The Dog Chapel. He's also a really cool artist. Lives in Vermont. There *is* actually a Dog Chapel he created up there. The book contains a 'memorial' part where you can send in a picture of your dog so that it will hang in the chapel that people visit.

I saw this at the bookstore last month. It's a book where you can write down your memories of your dog.
