Yep. Is there a recipe? Nope. Instructions? I'll try.
Start with some finely chopped white or yellow onion -- I use about 1/4 cup for sauce for 1-2 lbs of shrimp, depending on how heavy a dipper you are. Probably about a cup and a half of ketchup -- Heinz 57. Add a squeeze of lemon juice, a shake or two of Worchestershire sauce and the grated horseradish to taste (I may use half a bottle of prepared horseradish depending on the jar). You can taste test on a saltine or captain's wafer cracker.
If you buy prepared horseradish, use the type that has nothing more than a little water or vinegar added (maybe a little salt -- but no cream, sweetener or anything like that).

I'm pretty certain I've never tasted homemade so I'm going to try Beths easy instructions.
We like it really strong. I like the TJ's brand best, although sometimes I even add some horseradish to kick it up a bit.

I just use ketchup, horseradish to your liking (just keep tasting, til the heat is just right) and a splash of fresh lemon juice.
Don't forget to add a few drops of tabasco sauce if you like it hot.
1 Cup Ketchup
1-2 Tbsp. Horseradish (taste as you go along)
Several squirts Tabasco sauce
Juice of half lemon
